Is your child starting Kindergarten next school year?
KINDERGARTEN IS AN exciting and fun-filled year, and lays the foundation for academic, healthy social, and emotional development! Parents can register their children for MUSD Kindergarten classes between March 2 and March 13, 2020. School site staff will be available to answer questions and assist you with the registration process.
Parents must bring the following documents to register their child for Kindergarten. If you do not have a copy of these documents for registration, order these documents as soon as possible. Many states have online access to order birth certificates. Contact your child’s pediatrician to obtain immunization records.
- TB survey
- Immunization record
- Birth certificate
Parents must also complete the following forms:
- Registration form
- Health History form
- Home Language Survey, with date
Proof of residency is required, but can be provided closer to the beginning of the school year with any of the following documents:
- A rental agreement or purchase agreement on a residence
- Utility bills (telephone, gas, electric) that list the parent or guardian’s correct name and address
- A parent or guardian’s driver’s license and automobile registration
- An automobile or property insurance policy
- An income tax W-2 form
- Property tax bill
